To refer an injured worker to IVS, follow one of these easy steps:
OPTION 1. EASIEST AND FASTEST WAY TO VOC REHAB. |
INJURED WORKER CALLS MCO. IW TELLS MCO "I WANT VOC REHAB WITH IVS!"

OPTION 2. EASY BUT NOT AS FAST. |
AOR SENDS A FAX, LETTER, OR MEMO TO THE MCO REQUESTING VOC REHAB WITH IVS.
---OR POR COMPLETES C-9 REQUESTING VOC REHAB WITH IVS. (Please send copy to MCO and IVS) |
OPTION 3. ORIGINAL BUT STILL EFFECTIVE. |
COMPLETE THE IVS VOC REQUEST FORM INCLUDING THE INJURED WORKERS SIGNATURE. FAX OR EMAIL TO IVS AND THE MCO.

REMEMBER! YOU MUST ASK FOR IVS (INDEPENDENT VOCATIONAL SERVICES) TO GET ONE OF OUR SKILLED, EXPERIENCE AND CLIENT FOCUSED CASE MANAGERS. Injured workers with state funded employees still have the right to choose their voc rehab provider. Make sure you are helping your clients and patients choose IVS!
